Important Registration Notes
- Registration must be followed up by a letter from supervisor, advising of status on institutional letter head. Proof of status will be required at time of registration badge pick up.
- Registration fee applicable to additional personnel outside the committed sponsorship level, please refer to the sponsor prospectus for further information.
- Registrants wishing to participate in the Oncoplastic must have completed at minimum 20 breast cases in the last 12 months. All oncoplastic learners will be forwarded a survey link that must be completed as part of the workshop curriculum.
All registration fees are subject to 13% HST
Refunds will not be issued for cancellations received after June 15, 2017. A processing fee of $60 (plus 13% HST) will be retained on all cancellations.
Registrations are not transferable.
The University of Toronto reserves the right to cancel events. Registrants will be notified at the earliest possible date in the event of a cancellation. Registration fees for events cancelled by the University will be refunded; however the University will not be liable for any loss, damages or other expenses that such cancellations may cause.
